This Privacy Policy describes how CarifyOne, Inc. ("CarifyOne," "we," "us," or "our") collects, uses, and shares personal information when you visit carifyone.com and its related pages (the "Site"), request a demo, use the Savings Calculator, communicate with us, or otherwise interact with the Site.

This Privacy Policy does not apply to the CarifyOne practice management platform (the "Platform"). The Platform is provided to dental practices under written customer agreements, including a Business Associate Agreement where required by HIPAA, and those agreements govern how information in the Platform is handled. Please do not submit patient information or other protected health information through the Site.

By using the Site, you agree to the collection, use, and sharing of information as described in this Privacy Policy. If you do not agree, please do not use the Site.

1. Information We Collect

Information you provide to us. When you request a demo, contact us, use the Savings Calculator, subscribe to communications, or submit a testimonial or review, we collect the information you choose to provide, which may include:

• Contact and business details, such as your name, practice name, role, email address, phone number, city and state, the practice management software you currently use, and any message you send us.
• Savings Calculator inputs and results, such as what your practice pays for software and services, the number of providers, card processing volume and rates, staffing estimates, the plan you selected, and the comparison the calculator produces.
• Your consent choices, such as whether you agreed to receive text messages, and the date and time of that consent.
• Testimonials, reviews, and feedback you agree to share with us, and any other information you choose to send us.

Information collected automatically. When you use the Site, we and our service providers automatically collect certain information about your device and your use of the Site, including your IP address, device type, browser type and settings, operating system, referring website or advertisement, the pages you view, the links you click, the time spent on pages, scrolling and mouse movement, approximate location derived from your IP address, and identifiers associated with cookies and similar technologies. Section 4 describes the analytics, advertising, session replay, and chat technologies we use.

Information from other sources. We may receive information about you from business contact databases and sales prospecting tools (for example, publicly available professional information such as a practice name, address, role, and business email address or phone number), from partners and referrals, and from public sources, and we may combine it with information we collect on the Site.

Representative attribution. If you reach the Site through a link provided by a CarifyOne representative, we record that representative's name with your submission so we can route your request to the right person.

2. How We Use Information

We use the information we collect to:

• Respond to your demo requests and questions, and provide Savings Calculator results.
• Communicate with you about CarifyOne products, features, pricing, events, and offers, by email, phone, and, if you have opted in, text message.
• Operate, maintain, analyze, and improve the Site, and understand how visitors use it.
• Personalize content and measure and improve the effectiveness of our advertising, including by showing you CarifyOne advertisements on other websites and services.
• Detect, prevent, and respond to fraud, abuse, security incidents, and other harmful activity.
• Comply with legal obligations, enforce our terms, and protect our rights and the rights of others.
• Maintain business records and support our general business operations.
• For any other purpose described to you at the time of collection or to which you consent.

3. Text Messages

If you provide a mobile phone number and opt in to receive text messages, CarifyOne may send you text messages about demo scheduling, follow-up on your inquiry, and CarifyOne products and offers. Messages may be sent using automated technology. Message frequency varies. Message and data rates may apply. Consent to receive text messages is not a condition of purchasing any goods or services.

You can opt out at any time by replying STOP to any message. Reply HELP for help, or contact us using the information in Section 14. Text messaging is currently offered only to United States phone numbers. Wireless carriers are not liable for delayed or undelivered messages. Our Mobile Messaging Terms are set out in the Terms of Service.

4. Cookies, Analytics, and Advertising Technologies

We and our service providers use cookies, pixels, tags, software development kits, local storage, and similar technologies (collectively, "cookies") on the Site. These fall into the following categories:

• Essential cookies, which are necessary for the Site to function, such as remembering choices you make on a page.
• Analytics cookies, such as Google Analytics, which help us understand how visitors use the Site, which pages are popular, and how visitors arrive.
• Advertising cookies, such as the Meta Pixel and the LinkedIn Insight Tag, which allow us and these platforms to measure the performance of our advertising and to show CarifyOne advertisements to you on other websites and services based on your visit to the Site.
• Session replay and heatmap tools, which record how visitors interact with pages, including clicks, scrolls, and mouse movement, so we can improve the Site's design. We configure these tools to mask the contents of form fields where the tool supports it.
• Chat and support tools, which allow you to communicate with us from the Site.

Your choices. Most browsers let you block or delete cookies through their settings, although doing so may affect how the Site works. Google offers a browser add-on to opt out of Google Analytics, and Meta and LinkedIn each offer advertising preference controls within their services. Where required by law, we honor opt-out preference signals such as Global Privacy Control. You may also opt out of the use of advertising cookies through the "Your Privacy Choices" link in the Site footer or by contacting us. We do not currently respond to browser "Do Not Track" signals, for which no common standard has been adopted.

5. How We Share Information

We share personal information in the following circumstances:

• Service providers. We share information with vendors that perform services on our behalf, such as website hosting and infrastructure, email delivery, spreadsheet and document tools, customer relationship management and sales tools, analytics and advertising platforms, session replay tools, and communication tools. These providers are permitted to use the information only to provide services to us, except that analytics and advertising platforms may also use information as described in their own privacy policies.
• Professional advisors, such as lawyers, accountants, and insurers, where necessary for the services they provide to us.
• Legal and safety. We may disclose information to comply with law, regulation, legal process, or governmental request; to enforce our terms; to protect the rights, property, or safety of CarifyOne, our customers, or others; and to investigate fraud or security issues.
• Business transfers. In connection with a merger, acquisition, financing, reorganization, or sale of all or part of our business or assets, information may be transferred to the acquiring or successor entity.
• With your consent or at your direction.
• Aggregated or de-identified information that cannot reasonably be used to identify you.

We do not sell personal information in exchange for money. Because we use advertising cookies, our use of them may be considered "sharing" of personal information for cross-context behavioral advertising or "targeted advertising" under certain state privacy laws. You can opt out as described in Sections 4 and 8.

6. Testimonials and Reviews

We publish testimonials and reviews on the Site only with the approval of the person who provided them. A published testimonial may include the person's name, practice name, and location. If you have provided a testimonial and would like it updated or removed, contact us using the information in Section 14.

7. Data Retention

We retain personal information for as long as necessary for the purposes described in this Privacy Policy, including to maintain business records, resolve disputes, enforce our agreements, and comply with legal obligations. We may retain lead and contact information indefinitely unless you ask us to delete it. When you request deletion, we will delete or de-identify the information unless we are required or permitted by law to retain it.

8. Your Privacy Rights

Everyone. You may ask us to access, correct, or delete personal information we hold about you, and you may opt out of marketing communications at any time by using the unsubscribe link in an email, replying STOP to a text message, or contacting us. We may still send you non-marketing messages, such as responses to your requests.

United States state privacy rights. Depending on where you live, you may have the right to: confirm whether we process your personal information and access it; correct inaccurate personal information; delete personal information; obtain a copy of personal information in a portable format; opt out of the sale of personal information, the sharing of personal information for cross-context behavioral advertising, targeted advertising, and certain profiling; and not be discriminated against for exercising these rights. To exercise these rights, email us at matt@carifyone.com with the subject line "Privacy Request," or use the "Your Privacy Choices" link in the Site footer to opt out of advertising cookies. We may need to verify your identity before responding, which we generally do by matching the information in your request to the information we hold. You may designate an authorized agent to make a request on your behalf, and we may ask for proof of the agent's authorization. If we deny your request, you may appeal by replying to our decision, and we will explain the outcome of the appeal in writing.

California residents. In the preceding twelve months we have collected the following categories of personal information: identifiers (such as name, email address, phone number, and IP address); commercial information (such as Savings Calculator inputs and your interest in our products); internet or other electronic network activity (such as pages viewed and interactions with the Site and our advertisements); approximate geolocation derived from IP address; professional or employment-related information (such as your practice and role); and inferences drawn from the foregoing. We collect this information from you, from your devices, and from the other sources described in Section 1, for the purposes described in Section 2, and we disclose it to the categories of recipients described in Section 5. We do not collect sensitive personal information as defined by California law, and we do not knowingly sell or share the personal information of anyone under 16. We retain information as described in Section 7. California's "Shine the Light" law permits residents to request certain information about disclosures to third parties for their direct marketing; we do not make such disclosures.

Canada. If you are in Canada, we collect, use, and disclose your personal information with your consent or as otherwise permitted by law. You may withdraw consent, subject to legal and contractual restrictions, and may request access to or correction of your personal information by contacting us. Your information is stored and processed in the United States and is subject to United States law. You have the right to complain to the Office of the Privacy Commissioner of Canada.

European Economic Area, United Kingdom, and Switzerland. The Site is intended for visitors in the United States and Canada. If we process personal information about a visitor in these regions, we do so on the basis of your consent, our legitimate interests in operating and marketing our business, the performance of a contract, or compliance with a legal obligation. You may have the right to access, rectify, erase, restrict, or object to our processing of your personal information, to data portability, to withdraw consent, and to lodge a complaint with a supervisory authority. Information is transferred to and processed in the United States. To exercise these rights, contact us as described in Section 14.

9. Security

We maintain administrative, technical, and physical safeguards designed to protect personal information against unauthorized access, use, and disclosure. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

10. Children

The Site is intended for adults and business users. It is not directed to anyone under 18, and we do not knowingly collect personal information from anyone under 18. If you believe a person under 18 has provided us with personal information, contact us and we will delete it.

11. Third-Party Websites and Services

The Site may contain links to websites and services operated by others, including social media platforms and scheduling tools. Those services are governed by their own privacy policies, and we are not responsible for their practices.

12. International Transfers

CarifyOne is located in the United States, and the information we collect is stored and processed in the United States, where privacy laws may differ from those in your jurisdiction. By using the Site, you understand that your information will be transferred to and processed in the United States.

13. Changes to This Privacy Policy

We may update this Privacy Policy from time to time. When we do, we will post the updated version on the Site and change the "Last updated" date. If the changes are material, we will provide additional notice as required by law, such as by email or a notice on the Site. Changes take effect when the updated Privacy Policy is posted, unless a later effective date is stated at the time of posting.
